Your enrollment checklist
Use this checklist to make the most of this opportunity to enroll:
- Learn about your benefit options.
- Use the tools and resources available to help you make informed benefit decisions.
- Set up your Flexible Spending Account (FSA) enrollment and/or Health Savings Account (HSA) contribution amount.
- Make sure your dependent information is correct and all your dependents are eligible.
- Review Review and update your beneficiary information.
- Complete your benefits enrollment within 30 days of receiving your new hire enrollment or new benefits eligibility task in your Workday inbox or within 30 days of a qualifying life event. Learn how to enroll.
- Effective Dates
- If you are a new hire, coverage starts on the first of the month following 30 days of employment
- If you became newly eligible for benefits, coverage starts on the first of the next month following your transfer date
- If you have a qualifying life event, coverage starts on the first of the next month following the event date, except for newborns, who start on the date of birth
Eligibility

Health and income protection benefits
All active regular full time or part time associates who are scheduled to work 20 or more hours per week are eligible to participate in CHOC's health and income protection benefits. You may also cover your eligible dependents under CHOC's medical, dental, vision, supplemental medical and life benefits.
Your eligible dependents include:
- Spouse/California Registered domestic partner (same or opposite gender)
- Your child(ren) and the child(ren) of your covered spouse/California Registered domestic partner (up to age 26)
- Children with disabilities who became disabled on or before age 26
Please note: When enrolling your spouse/domestic partner or child(ren), you will be required to provide proof of marriage, California domestic partner registration and/or birth certificates. If you enroll your spouse/domestic partner in medical coverage and they have medical coverage available through an employer, a $50 per pay period surcharge will be added to your medical plan premiums.
ACA Eligibility
Under The Affordable Care Act (ACA), you are eligible for coverage if you have worked on average either a) 30 hours or more per week or b) 130 hours per month in a “look back period,” which is October 1 – September 31. If your status changes and you no longer are benefits eligible, all benefits coverage will cease, except for your enrollment in a medical plan. You will remain covered under the medical plan through the end of the calendar year, unless you elect to cancel this coverage in Workday. If you waive your ACA medical coverage and become benefits eligible again in the same year, you will not be able to enroll in medical coverage. CHOC 403(b) Plan
You may enroll at any time after your hire date into the CHOC 403(b) Plan and may contribute between 1% and 85% of your eligible pay to your retirement plan account, up to annual IRS limits. You may change your contribution rate and investment elections at any time by going to http://www.mychocretirement.com.

How to Enroll
Health and income protection benefits
You must enroll within 30 days of receiving the new hire enrollment task in your Workday inbox or within 30 days of your transfer to a benefits eligible position or returning from a leave of absence. If you experience a qualifying life event, you must make changes to your benefits coverage within 30 days of the event to have coverage this year. Enroll or make changes in Workday. For a new hire, coverage takes effect on the first of the month following 30 days of employment. Supplemental life insurance, STD and LTD subject to Evidence of Insurability (EOI), will become effective the day The Hartford approves the coverage. For qualifying life events or transfers to a benefits eligible position, coverage takes effect the first of the month following the date of the event, with the exception of supplemental life insurance, STD or LTD. For births, coverage takes effect as of the date of birth. Transfers from a benefits eligible position to another benefits eligible position is not a QLE, and is not an opportunity to change or add benefits.
Log in and make your elections
You will enroll or make changes to your benefits in Workday. If you are a new hire or newly eligible for benefits coverage, look for a task in your Workday inbox to begin the process. If you want to make changes due to a qualifying life event, go to the Benefits and Pay app on your Workday homepage. Once you have completed enrolling or making changes, you must click the orange Review and Sign tab, then scroll down and check I Accept and the Submit tab to finalize your elections. Please note: It is recommended that you print and save a copy of your benefit elections for your records.

- Be sure to provide the necessary dependent information to enroll eligible family members
- Make sure you’ve input your beneficiaries for your life insurance. Review the Job Aid for designating a beneficiary on the Documents page
Don't miss out
If you don’t enroll within 30 days of receiving your new hire enrollment or newly benefits eligible enrollment task in Workday, you cannot make benefit elections until our Open Enrollment period, unless you experience a qualifying life event. CHOC 403(b) Plan
You have two ways to enroll in the CHOC 403(b) Plan to invest for retirement through easy payroll deductions:
- Online – Enroll on the Empower website any time of day or night
- By Phone – Call 833-246-2403 if you have questions or need assistance. Representatives are available to help you from Monday – Friday 5 a.m. – 7 p.m.; Saturdays 6 a.m. – 2:30 p.m.
