Age milestone

Bereavement

Loss or gain of benefit coverage
(you or a dependent)
  • Enroll or change your benefits coverage, if necessary, within 30 days of gaining or losing other benefits coverage.
  • If are enrolled in the medical plan, you may be eligible to continue this coverage under ACA even though you lose your other benefits coverage. Contact the Benefits Department to find out.
  • If losing coverage of a CHOC health plan, look to receive a COBRA packet from HealthEquity/WageWorks.

  • Update your state tax withholding information, if necessary, in Workday.

This may be a qualifying life event, if you are moving to or from a location where your current medical plan does not cover your new location. When you experience a qualifying life event, such as moving your residence, you have 30 days from the date of your event to make benefits changes. Visit Workday to adjust your benefits coverage.

Natural Disasters

Pets

Pregnancy

Promotion
  • Adjust your benefits coverage, if your eligibility for benefits has changed, within 30 days of your work status change.
  • Schedule an appointment to speak to a financial advisor about your retirement account investments and other financial concerns.
  • Speak to Empower for help in updating your retirement plan contributions.
  • Order new business cards, if necessary.

This may be a qualifying life event if the promotion makes you newly eligible to enroll in benefits. When you experience a qualifying life event, such as a change in your work status, you have 30 days from the date of your event to make benefits changes. Visit Workday to adjust your benefits coverage.

Retirement
  • Notify your manager.
  • Review the company’s retiree medical program and contact Benefits Department if you are eligible and want to enroll in coverage.
  • Learn about your options for receiving Social Security benefits, if eligible.
  • Apply for Medicare, if eligible.
  • Speak to Empower for help in deciding when you may access your retirement plan savings.
  • Convert supplemental life insurance to individual policy, if desired, once you receive a conversion notification from The Hartford.
  • Access free, confidential counseling through the Employee Assistance Program (EAP).

After you’ve retired, you may be eligible for Medicare and/or Social Security benefits. Learn more on the Social Security website. You may also have the option of enrolling in COBRA coverage or your spouse’s benefits. Ask your spouse’s employer for their enrollment process and deadline.

Societal Events

This is not a qualifying life event. You are not able to change your benefits coverage due to this event.

Stress/Burnout

This may be a qualifying life event if you take a leave of absence. When you experience a qualifying life event, you have 30 days from the date of your event to make benefits changes. Visit Workday to adjust your benefits coverage.

Termination of Employment
  • Review the company’s Resignation of Services Policy.
  • If losing coverage in a CHOC health plan, look to receive a COBRA packet from HealthEquity/WageWorks within 2 weeks after termination.
  • Schedule an appointment to speak to a financial advisor about your retirement account investments and other financial questions.
  • Supplemental life insurance ends on the termination date. Convert supplemental life insurance to individual policy, if desired, once you receive a conversion notification from The Hartford.
  • All accrued, unused PTO will be paid in your last paycheck in accordance with the company’s Paid Time Off Policy.
  • Paid Sick Leave will not be cashed out in accordance with the company’s Paid Sick Leave Policy.
  • Continue MetLife Legal plan through the portability option, if desired.
  • Convert Nationwide Pet Insurance to an individual policy by completing a Continuation of Coverage form sent to you from Nationwide after termination.
  • You have 90 days from the termination date to utilize any unused funds in a Health Care Flexible Savings Account (HCFSA) with WageWorks for health care expense that occurred prior to termination.
  • Any funds in a Health Savings Account will remain yours.
  • Separation of employment or a termination event with a future date entered into Workday before a tuition reimbursement is processed will be cancelled in accordance with the Education Reimbursement Policy.
  • Short-Term Disability and Long-Term disability coverage with The Hartford ends on termination date.
Vacation

This is not a qualifying life event. You are not able to change your benefits coverage due to this event.
