CHOC offers coverage to help protect your income in the event of a disability.

Overview

A serious illness or injury can cause significant financial hardship for your family. Disability insurance, provided through The Hartford, replaces a portion of your income to help you continue paying your bills and meeting your financial obligations during this difficult time.

Short-term disability

You may choose to purchase coverage that pays you a disability benefit for up to 24 weeks.

Long-term disability

Long-term coverage protects you from the financial impact of an extended disability.

How to file a claim

When you’re absent from work, The Hartford can help you decide when to file a short-term or long-term disability claim.

  • Contact The Hartford 30 days prior to a scheduled absence from work. If your absence is unplanned, call as soon as possible
  • When you call, have the following on hand: your name, address, name of your employer, your last full day of active work, the nature of your claim, and your treating physician’s contact information
  • Call The Hartford at 888-899-3392 or go online to file a claim

Short-Term Disability

Short-term disability coverage provides you with:

  • Benefits that start after two weeks of disability.
  • Payments equal to 70% of your salary, up to a maximum of $1,500 per week.
  • Up to 24 weeks of benefits.

California state disability insurance and your short-term disability payments

Associates residing in California are eligible for short-term disability benefits through the California State Disability Insurance (CASDI) program. CASDI is funded through employee payroll deductions and provides short-term benefits to eligible workers who become injured or suffer an illness and cannot perform the duties of their job. Any benefits paid under this policy will be subject to income tax.

While you may choose to purchase short-term disability coverage, this benefit is reduced by the amount of CASDI payments you receive.

Long-Term Disability

Long-term disability coverage provides you with:

  • Benefits that start after you have been disabled for 180 consecutive days
  • Payments equal to 60% of your regular monthly earnings up to a maximum of $5,000 per month

Benefits end when your disability ends or you reach age 65. If you are between the ages of 60 and 65 when you become disabled, benefits will be paid up to a maximum of 60 months. If you are between the ages of 66 and 69 when you become disabled, your benefits will be paid up to a maximum of age 70.
